TELEHEALTH CONSULTATION

SCHEDULE A TELEHEALTH CONSULTATION

Please contact us through the secure email on this website:

[email protected]

Please email from the web address where you want to be called for your Zoom call. If you prefer to talk by telephone, please provide the number to call. Also provide a telephone number where you can be reached to schedule your telehealth consultation.

PLANNING YOUR TELEHEALTH CONSULTATION

Some forms you can fill out before your consultation [click link to office forms in the Steps You Can Take section].  Other forms need to be completed and signed before your consultation.

Telehealth consultation is not medical care in the usual sense. Thorough medical care would require in person examination. Rather, telehealth consultation is an opportunity to provide the information and perspective that enables you to make personal health decisions that are consistent with your values and your goals.

It is always useful to think about the goals of your consultation before it begins.

Please feel free to send a list of questions before your consultation.

If you would like to ask about recent news you have seen or studies you have heard about, please send the article or material before your consultation.

In general, we consult with women for one of three reasons:

1.  Women who have been diagnosed with breast cancer.  If you have been diagnosed with breast cancer, please consider these questions.

2.  Women who have a specific area of concern or symptom including a breast lump, abnormal mammogram, pain, discharge or other concern.  If you are scheduling your consultation for these reasons, please consider these questions.

3.  Women who are concerned that they are at high risk for breast cancer or want to minimize their risk of breast cancer. If you are scheduling your consultation for these reasons, please consider these questions.

URGENT CARE

WE DO NOT PROVIDE URGENT CARE.  WE PROVIDE ONLY NON-EMERGENCY CONSULTATIONS CONCERNING YOUR HEALTH DECISIONS.  WE MONITOR THE EMAIL AND WILL TRY TO RESPOND TO REQUESTS WITHIN 48 HOURS, MONDAY THROUGH FRIDAY.

WE ARE NOT AVAILABLE TO RESPOND ON WEEKENDS

If you have a life-threatening emergency, please call 911 or go to the nearest Emergency Room. It is important that you contact your personal physician within 24 hours of receiving emergency care.

The office email is for arranging telehealth consultations only. It is not intended for patient care. Do not send patient care information such as results of tests unless it is requested.

No medical advice will be given by email.
